Admissions
When a resident is transferring to the Facility from a hospital or other nursing facility, the following items will be needed at the time of admission:
- Hospital transfer form
- Medical history and physical examination with the previous five days
- Laboratory reports (blood and urinalysis)
- Chest X-ray performed no sooner than 90 days prior to admission
- Current OBRA Screening forms
Please ask the social worker or discharge planner to help you obtain these forms. We cannot, as a matter of policy, admit a resident from a hospital or other facility without them.
When a residents is being admitted from home we require:
- Medical history and physical examination with the previous five days
- Chest X-ray performed no sooner than 90 days prior to admission
- Physician's order for medication, diet, activity limitations, etc.
- Current OBRA Screening forms
It is our policy that each resident have a guarantor prior to admission. The guarantor is a person other than the resident who agrees to be financially responsible for the resident's funds. This is usually a family member, although arrangements can be made with a bank trust officer if you prefer.
From the guarantor, the following items are needed on or before the day of admission:
- Complete admission and social history records at this facility
- A financial agreeement signed by both the resident and the guarantor
- For residents not covered by Medicare or HMO, a deposit in advance of 30 times the established daily rate
- The resident's Medicare, HMO or other insurance card
