Admissions

When a resident is transferring to the Facility from a hospital or other nursing facility, the following items will be needed at the time of admission:

  1. Hospital transfer form
  2. Medical history and physical examination with the previous five days
  3. Laboratory reports (blood and urinalysis)
  4. Chest X-ray performed no sooner than 90 days prior to admission
  5. Current OBRA Screening forms

Please ask the social worker or discharge planner to help you obtain these forms. We cannot, as a matter of policy, admit a resident from a hospital or other facility without them.

When a residents is being admitted from home we require:

  1. Medical history and physical examination with the previous five days
  2. Chest X-ray performed no sooner than 90 days prior to admission
  3. Physician's order for medication, diet, activity limitations, etc.
  4. Current OBRA Screening forms

It is our policy that each resident have a guarantor prior to admission. The guarantor is a person other than the resident who agrees to be financially responsible for the resident's funds. This is usually a family member, although arrangements can be made with a bank trust officer if you prefer.

From the guarantor, the following items are needed on or before the day of admission:

  1. Complete admission and social history records at this facility
  2. A financial agreeement signed by both the resident and the guarantor
  3. For residents not covered by Medicare or HMO, a deposit in advance of 30 times the established daily rate
  4. The resident's Medicare, HMO or other insurance card
